How to Report Identity Theft to the FTC

Click on the statement that best describes the incident you faced.

Click on any additional details that best describe your situation.

Continue to click on the details that best describe your situation.

Fill out all the details about the incident that you can.

You will be asked when you first noticed the problem and the total amount of fraudulent charges, if any. Continue to fill out the details on this page.

Enter in details about anyone you spoke to, if applicable.

Once you have finished with this page, click Continue.

Enter your full name, phone number and phone type. NOTE: These are the only pieces of contact information you need to enter before continuing. Only enter pieces of contact information you feel comfortable sharing

If you have any information about the person who stole your identity, click yes.

Enter their name and address, if you have it.

Enter their contact information and how you suspect they got your information.

If you have reviewed a copy of your credit report, click yes.

Click on all the options that are applicable to your situation.

If you have requested a fraud alert from one of the three national credit monitoring bureaus, click yes.

Enter the date you placed the fraud alert.

If you have contacted your local police department about the crime, click yes.

Enter the name of the police department and their where they are located.

If your information has been exposed in a data breach, click yes.

Choose one of the three options listed here.

If a debt collector has contacted you about an account that isn't yours, click yes.

Once you are done, click continue.

On this page, explain the situation you faced. NOTE: Do not enter in any sensitive personal information.
